The explanation is provided by the Ministry of Education and Science of Ukraine.
To obtain a duplicate of a higher education document and/or its supplement, you must submit a written application to the university that issued the document. This can be done in person or through an authorised representative.
If the institution where you studied has been reorganised, you should contact its successor. This refers to cases of accession, merger or liquidation of a higher education institution.
If there is no legal successor or the HEI does not carry out educational activities (the licence has been cancelled, it has not been moved from the uncontrolled territory), the application must be submitted to the Ministry of Education and Science. The Ministry will determine the institution that will issue a duplicate.
Provide thee following information in the application:
- full name, date of birth;
- series and number of the identity and citizenship document;
- place of residence, telephone number;
- name of the higher education institution and date of graduation;
- name of the document whose duplicate is ordered;
- reason for ordering a duplicate;
- other information that you consider important;
- consent to the processing of personal data.
Applications to the Ministry of Education and Science of Ukraine should be sent to the address: 10 Peremohy Ave, Kyiv, 01135 or to ez@mon.gov.ua.
The information in the duplicate is reproduced on the basis of the data contained in the Unified State Electronic Database on Education. Information about documents has been entered into the database by higher education institutions since 2000.
If information about the fact of issuing a diploma is not available in the Unified State Database of Education Records, the HEI may use copies of the document, as well as an archival certificate of study and an extract from the journal of issuing documents on higher education or an act of destruction of the primary document on higher education to confirm this.
If information about the fact of issuing a diploma is not available in the Unified State Database of Educational Records and the archives of the higher education institution are not available, then it is possible:
- apply to the court – it must establish the legal fact of obtaining the relevant degree or level of higher education;
- confirm the receipt of the diploma by an official letter from the relevant competent authority, which confirms the fact of affixing the “Apostille” stamp of the Ministry of Education and Science of Ukraine or consular legalisation.
Therefore, it is necessary to submit an application to the Ministry of Education and Science to determine the institution that will issue a duplicate. The application must be accompanied by a copy of the court decision or an official letter confirming the apostille.
If you need to restore a diploma obtained in the territory of Crimea or Donbas not controlled by Ukraine before 2000, the procedure is the same as described above. If it is a question of making a duplicate of a document issued from 2000 to 2014 and entered in the USES, then:
- applicants from Crimea – must submit a written application to the Ministry of Education and Science to determine the institution that will issue a duplicate;
- applicants from Donbas should submit a written application to the successor or the HEI relocated from the uncontrolled territory. Otherwise, they must submit a written application to the Ministry of Education and Science to determine the institution that will issue a duplicate.
A copy of the primary document (diploma and its supplement) must be attached to the application.
